29 CFR 1910.184(c)(10): Hands or fingers were placed between the sling and its load while the sling was being tightened around the load:   a) On or about January 29, 2025, at the Cut to Length Department: employees were exposed to amputation hazards, while hoisting and moving 2 pcs 5'-0" long of W24x104 beams with a weight of 1,040 lbs., using a Budget Chain Hoist Serial no. 298226/Model no. BEH0208, in that, proper rigging procedures were not followed.

29 CFR 1904.39(a)(2): The employer did not report within 24 hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about January 29, 2025, Equipment Fabricators, Inc: the employer failed to report to OSHA within 24 hours a work-related amputation of an employee. A law enforcement officer notified OSHA on February 4, 2025.
