Standards in this part requiring training on hazards and related matters, such as standards requiring that employees receive training or that the employer train employees, provide training to employees, or institute or implement a training program, impose a separate compliance duty with respect to each employee covered by the requirement. The employer must train each affected employee in the manner required by the standard, and each failure to train an employee may be considered a separate violation. a) On or about February 14, 2025, the employer failed to ensure that an employee operating a powered industrial truck was sufficiently trained to recognize and avoid a pinch point hazard created when removing a debris hopper from the product dump area. Specifically, the employee was not sufficiently trained to recognize a pinch point and to keep their hands out of that area, which resulted in exposure to crushing and amputation hazards.
