29 CFR1910.178(a)(4): Modifications or additions which affect capacity and safe operation of a powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly. Location: 1901 Commerce Drive Bridgeport, CT 06605 On or about February 25, 2025 and times prior to, the employer modified a Hyster LPG powered industrial truck's capacity and safe operation by adding a Aardwolf Model PFB2-3450 telescoping jib to move granite and quartz slabs without the manufacturer's prior written approval.
