Location- 20 Bayberry Rd, Natick, MA  On February 25, 2025, employees were exposed to fall hazards of approximately 20 feet while working from a scaffold on a residential construction project where a competent person qualified in scaffold erection was not present to supervise or direct the scaffold erection
