29 CFR 1910.22(a)(1):  Surface conditions. The employer must ensure:  All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  (a)  On or about February 26, 2025, the employer failed to ensure the walking-working surfaces inside the facility were free of debris, such as but not limited to plastic, cardboard, paper scraps, wood scraps, boxes of trash, and overflowing garbage cans exposing employees to the possibility of slip, trip, and fall injuries.

29 CFR  1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.   (a)  On or about February 26, 2025, the employer did not ensure that portable fire extinguishers were mounted, located, identified, and unobstructed, at least six fire extinguishers in the receiving area were observed to be blocked by trash and empty boxes.

29 CFR  1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.   (a)   On or around February 26, 2025, the employer failed to ensure that employees who were allowed to use fire extinguishers for extinguishing incipient stage fires received training on the general principles of fire extinguisher use and the hazards associated with fighting such fires.

29 CFR 1910.303(g)(1):  Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  (a)  On or about February 26, 2025, the 240 volt electrical panels that provided electrical power to the facility were blocked by boxes of merchandise, which prevented safe operation and maintenance of the equipment.

29 CFR 1910.37(a)(3):  The danger to employees must be minimized.  Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  (a)  On or about February 26, 2025, the employer did not ensure that exit routes were free and unobstructed, at least six exit doors in the receiving area were observed to be blocked by trash and empty boxes.
