On or about February 23, 2025, and at times prior to, within the main dining area of the restaurant, employees were exposed to fire hazards when a propane torch kit was used to light the wood of an indoor enclosed firepit; and,   b.) On or about February 25, 2025, and at times prior to, within the bar dining area of the restaurant, the employer was using multiple propane gas powered patio heaters, listed for outdoor use only, inside of the building.

On or about February 25, 2025, and at times prior to, in front of the kitchen to the north wall, a stairway used by employees to access the attic, did not have hand rail or stair rail systems installed.  b.)  On or about February 25, 2025, and at times prior to, within the west side of the building near the bar area of the dining room, employees access the second floor seating area using a staircase with a stair rail system that does not have a mid-rail.

Cast iron must not be used for container valves, fittings, and accessories. This does not prohibit the use of container valves made of malleable or nodular iron.  On or about February 25, 2025, and at times prior to, behind the southeast side of the restaurant, employees use a 1000-gallon propane tank to refill smaller portable propane tanks and this 1000-gallon propane tank had accessories that were not made of a material and design suitable for LP-Gas service.

On or about February 25, 2025, and at times prior to, behind the southeast side of the restaurant, during bottle refilling operations employees use a 1000-gallon propane tank to refill smaller portable propane tanks and the 1000-gallon propane tank did not have an excess flow valve or a backflow check valve to prevent an uncommanded discharge of propane.
