On or about February 23, 2025, and at times prior to, within the main dining area of the restaurant, employees were exposed to fire hazards when a propane torch kit was used to light the wood of an indoor enclosed firepit; and, b.) On or about February 25, 2025, and at times prior to, within the bar dining area of the restaurant, the employer was using multiple propane gas powered patio heaters, listed for outdoor use only, inside of the building.

On or about February 25, 2025, and at times prior to, behind the southeast side of the restaurant, during bottle refilling operations employees use a 1000-gallon propane tank to refill smaller portable propane tanks and the 1000-gallon propane tank did not have an excess flow valve or a backflow check valve to prevent an uncommanded discharge of propane.
