29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and caught-in-between hazards:  a) Bifulco's Four Seasons Cold Storage Inc. - 590 Almond Road, Pittsgrove NJ- G Room Cooler:  Employees actively stocked product weighing approximately 1500 lbs. per pallet, onto industrial steel racking systems  with damaged and out of plumb vertical supports, damaged column base plates, and sheered or broken off anchor bolts on the base plates, thus exposing the employees to struck-by and caught-in-between hazards.  Damaged racks included but were not limited to sections GR 501 B, GR 501 M, GR 501 T, GR 615 B, GR 614 B. This condition was observed on or about 02/27/2025.

29 CFR 1910.178(g)(2):  The battery charging apparatus for industrial trucks located in battery changing and charging installations was not protected from damage by trucks:  a) Bifulco's Four Seasons Cold Storage Inc. -  Forklift charging area: The facility did not protect the battery charging apparatuses from damage.  The charging cables on the North East battery charger Model# 3NE24-900, S/N 24541I 23 had damaged cords with damaged outer casing, observed on or about 02/27/2025.

29 CFR  1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:  a) Bifulco's Four Seasons Cold Storage Inc. -  Warehouse Room L:  The employer did not require forklift drivers to look in the direction of travel when reversing.  Employees relied only on the forklift's side mirrors and continued driving with limited visibility, leading to an incident in which an employee was run over, occurred on or around 02/26/2025.  b) Bifulco's Four Seasons Cold Storage Inc. -  Warehouse Room next to the packing area: The employer did not require forklift drivers to look in the direction of travel when reversing. Employees relied solely on the forklift's side mirrors and continued to drive through entrances where visibility was obstructed by a dirty plastic strip curtain, putting employees at risk of being run over, observed on or about 02/27/2025.
