On or about March 03, 2025,  and at times prior thereto, two employees engaged in metal panel installation on balcony dividers located approximately 10-100 feet above the ground below were connected to the same anchor point as part of their fall protection systems. The K-Strong UAF30090 and Werner A510000 anchors the employees were using were only rated for 5, 000 pounds per person attached, and exposed the employees to falling hazards.
