29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a. On or about March 27, 2025, an employee performing hoisting operations from the upper level roof was exposed to the hazard of free falling more than 6 feet and contacting lower level areas while using personal fall arrest systems that were not rigged to prevent these hazards.

On or about March 27, 2025, Anchorages being used for attachment of personal fall arrest equipment were not installed so that they would be capable of supporting at least 5,000 pounds per employee attached.
