29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a)Precision Grading, LLC at Sheridan Lake Road and Corral Drive, Rapid City, SD 57702:  On or about and at times prior to March 31, 2025  the employer did not ensure employees had an adequate means of egress every 25 feet while working within an excavation. This condition exposed employees to egress hazards.

This condition exposed employees to struck-by hazards.

29 CFR  1926.651(k)(2):Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:  a)Precision Grading, LLC at Sheridan Lake Road and Corral Drive, Rapid City, SD 57702:  On or about and at times prior to March 24, 2025  the employer did not ensure employees were protected from cave-ins as a result of employees working outside the protective system and the competent person did not remove employees from the hazard area.  This conditions exposed employees to cave-ins and crushing hazards.

29 CFR  1926.652(g)(1)(ii):Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:  a)Precision Grading, LLC at Sheridan Lake Road and Corral Drive, Rapid City, SD 57702:  On or about and at times prior to March 28, 2025  the exposing employer did not ensure that a trench box was adequately installed against the vertical walls within a trench in the event if lateral movement were to occur. This conditions exposed two employees to struck-by hazards if the trench box were to move.
