29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about March 25, 2025, establishment located at 1045 Lindoefer Road, Sheboygan, Wisconsin, in the Extrusion Department; employee(s) were exposed to machine hazards associated with moving parts when they engaged in product manipulation activities during set up on the CDS Puller, WP 338, Model: CBH36-12S, S/N: 51672 311121, located on Line 11. The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to manipulate the parts' orientation. The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity exposing employees to caught in between hazards from moving equipment. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Wisconsin Plastic Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.147(d)), which was contained in OSHA inspection number 1768639, citation number 1, item number 1 a and was affirmed as a final order on October 9, 2024, with respect to a workplace located at 1045 Lindoefer Road, Sheboygan, Wisconsin.

29 CFR 1910.212(a)(1): Machine Guarding. Types of guarding.�One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are - barrier guards, two-hand tripping devices, electronic safety devices, etc. At an establishment located at 1045 Lindoefer Road, Sheboygan, Wisconsin, in the Extrusion Department; (a) On or about March 26, 2025, on Line 12 on the CDS puller, WP 335, CDS, Model: CCH 40-70S, S/N: 140904, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (b) On or about March 26, 2025, on Line 15 on the CDS puller, WP 340, Model: CBH36-8V, S/N: 59040 211223, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (c) On or about April 1, 2025, on Line 13 on the Metaplast puller, WP 323, Model: PB-8-CL-SD, S/N: 698022, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (d) On or about April 2, 2025, on Line 14 on the CDS puller, WP 332, Model: CBH 36-8S, S/N: 331106, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (e) On or about April 2, 2025, on Line 2 on the CDS puller, Puller WP 334, Model: CCH 72-15I, S/N: 15771 22 11 11, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (f) On or about April 2, 2025, on Line 3 on the CDS puller, WP 326 Model: CBH 36-12 S, S/N: 261000, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (g) On or about April 2, 2025, on Line 4 on the Metaplast puller, WP 324, Model: PB-8-CL-SD, S/N: 799025, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (h) On or about April 2, 2025, on Line 5 on the Metaplast puller, WP 322, Model: PB6 CL SD, S/N: 596041, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (i) On or about April 2, 2025, on Line 6 on the CDS puller, WP 327, Model: CBH 36-12 S, S/N: 271000, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (j) On or about April 2, 2025, on Line 7 on the CDS puller, WP 333, Model: CBH 36-8S, S/N: 190708, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (k) On or about April 2, 2025, on Line 8/R & D Line on the CDS puller, WP 341, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (l) On or about April 2, 2025, on Line 9 on the CDS puller, WP 321, Model: PB6CL SD, S/N: 90113, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (m) On or about April 2, 2025, on Line 10 on the CDS puller, WP 339, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards. (n) On or about April 2, 2025, on Line 11 on the CDS puller, WP 338, Model: CBH36-12S, S/N: 51672 311121, employees conducting minor adjustments to the product prior to entering the puller were exposed to caught-in hazards created by the two in running conveyors belts. The machine was not guarded to prevent exposure to these hazards.
