29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about March 19, 2025, and times prior, at the worksite located at 4220 Commonwealth Drive, St. Joseph, Missouri, an employee was injured, hospitalized for approximately 20 days, and subsequently died of his injuries due to a fall at the site. The employer failed to report the hospitalization to OSHA within twenty-four (24) hours.
