29 CFR  1904.4(a):The employer did not record on the OSHA Form 300 or equivalent each work-related fatality, injury, or illness case that resulted in the general recording criteria:  On or about March 19, 2025, and times prior, at the worksite located at 4220 Commonwealth Drive, St. Joseph, Missouri, on or about April 3, 2025, an employee was pronounced dead as a result of injuries sustained at the worksite on March 19, 2025. The employer failed to record the incident on an OSHA Form 300 or equivalent form.

29 CFR  1904.29(b)(2): The employer did not complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log:   On or about March 19, 2025, and times prior, at the worksite located at 4220 Commonwealth Drive, St. Joseph, Missouri, the employer did not complete an OSHA 301 Incident Report form, or equivalent form following a recordable injury that occurred on or about March 19, 2025.

29 CFR  1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.    On or about March 19, 2025, and times prior, at the worksite located at 4220 Commonwealth Drive, St. Joseph, Missouri,  an employee was injured, hospitalized for approximately 20 days, and subsequently died of his injuries due to a fall at the site. The employer failed to report the hospitalization to OSHA within twenty-four (24) hours.
