29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 09, 2025,  on the north side of the building, an employee was not in use of fall protection while performing metal trim activities at heights greater than six (6) feet.  Betta Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13) , which was contained in OSHA inspection number 1616812, citation number 01, item number 01  and was affirmed as a final order on 11/23/2022, with respect to a workplace located at 10890 South 208th St. Gretna, NE 68028.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 09, 2025,  on the north side of the building, an employee performing siding work activities was using a portable ladder to access the upper landing surface and the side rails did not extent at least three (3) feet above the landing surface.
