Location: a) Worksite; 55 Livingston Ave, Roseland, NJ 07068; Parking lot area  a) On or about 4/14/2025, an employee performed demolition activities from an aerial lift without the use of fall protection and was exposed to a fall of approximately 14 feet from the basket of the aerial lift to the concrete surface below.

29 CFR  1926.454(c)(3):The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency  Location: a) Worksite; 55 Livingston Ave, Roseland, NJ 07068; Parking lot area  a) On or about 4/14/2025, an employee performed demolition activities from an aerial lift without the use of fall protection and was exposed to a fall of approximately 20 feet from the basket of the aerial lift to the concrete surface below. The employee did not retain the requisite proficiency in understanding the need and use of fall protection on an aerial lift.
