29 CFR  1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:   a) For Engine Room 1, on or about April 22, 2025, the employer failed to document compliance with recognized and generally accepted good engineering practices to protect employees exposed to anhydrous ammonia hazards as specified in ANSI/IIAR 2-1992 Standard for Safe Design of Closed-Circuit Ammonia Refrigeration Systems section 4.3.5, in that, supervised alarms were not in place for the failure of the mechanical ventilation system.    b) For Engine Room 1, on or about April 22, 2025, the employer failed to document compliance with recognized and generally accepted good engineering practices to protect employees exposed to anhydrous ammonia hazards as specified in ANSI/IIAR 2-1992 Standard for Safe Design of Closed-Circuit Ammonia Refrigeration Systems section A.4.1.1, in that, the Relief Header/Discharge terminated only 6.7 feet above the roof of the building that was accessed by employees.    c) For Engine Room 1, on or about April 22, 2025, the employer failed to document compliance with recognized and generally accepted good engineering practices to protect employees exposed to anhydrous ammonia hazards as specified in ANSI/IIAR 2-1992 Standard for Safe Design of Closed-Circuit Ammonia Refrigeration Systems section 4.4.11, in that, emergency remote controls located immediately outside the engine room were not in place to stop the action of refrigeration compressors.
