29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about April 24, 2025, employees which conduct maintenance/repair activities on equipment with various energy sources were not provided with specific procedures for each piece of equipment, detailing the energy sources and lockout/tagout isolation device for each of those hazardous energy sources. This exposed employees to crushing, caught-in and amputation hazards.
