29 CFR 1910.219(c)(4)(i):Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: (a) Pack Ops Area; On or about April 28, 2025, while working next to metal detector Loma IQ3, S/N KIMD20742, employees were exposed to sharp edges and catch points on the pulley shaft end collars. (b) Pack Ops Area: On or about April 28, 2025, while working next to metal detector Loma IQ3, S/N BMD22972-17832D, employees were exposed to sharp edges and catch points on the pulley shaft end collars. (c) Pack Ops Area: On or about April 28, 2025, while working next to metal detector Loma IQ3, S/N BMD23200-18781D, employees were exposed to sharp edges and catch points on the pulley shaft end collars. Additionally, the unguarded, 1 inch shaft ends projected 1 inch, which was more than half the diameter of the shaft.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Pack Ops Area; On or about April 19, 2025, an employee's right index fingertip was amputated while performing scheduled maintenance on the chain drive of a Loma Model IQ3 metal detector (Serial No. KIMD20742), which was not reported to OSHA until April 21, 2025.
