29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about April 25, 2025 and May 24, 2025, employees were exposed to hazards while working at the Interstate 55 Sangamon River Bridge Deck Replacement Project and the employer had not ensured a Competent Person conducted an inspection of the jobsite to identify and eliminate hazards, including but not limited to fall hazards and amputation injuries.

29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  On or about May 24, 2025, an employee was exposed to amputation injuries when operating a battery powered table saw to rip cut a bevel into 2-inch by 4-inch dimensional lumber at the Interstate 55 Sangamon River Bridge Deck Replacement Project and the employer had not ensured an adequate blade guard was installed.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about April 25, 2025, an employee was exposed to fall hazards approximately 21 feet to a lower level when working at the Interstate 55 Sangamon River Bridge Deck Replacement Project and the employer had not ensured the use of a fall protection system during the construction of of concrete deck forms.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  The employer failed to report to OSHA the following instances  at the Interstate 55 Sangamon River Bridge Deck Replacement Project within 24 hours:  1. On April 25, 2025, an employee was admitted to a medical facility for inpatient treatment of injuries sustained from falling 21 feet 6 inches to a lower level. 2. On May 24, 2025, an employee sustained an amputation injury to the left thumb after contact with a rotating table saw blade.
