29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables shall not be used.  a) Jobsite located at 1238 Belmont Ave, Philadelphia, PA the employer did not ensure that flexible cord sets were inspected for external defects prior to each shift or use.  Employees used flexible cord sets with external defects including but not limited to damaged wire insulation, thereby exposing employees to electrical hazards.  Condition was observed on or about May 12, 2025

29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a) Jobsite: 1238 Belmont Ave Philadelphia, PA  - The employer did not ensure that the scaffold planks employees were working from were at 18 inches wide, thereby exposing employees to a fall hazards of approximately 10 feet.
