29 CFR  1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  On or about May 8, 2025, an employee was injured while conducting housekeeping work in house 2. The employee fell into one of the four open holes, which measured 62 inches in length, 30.5 inches in width and 43.5 inches in depth. The fall occurred due to no fall protective system in place.
