29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a)S&T Construction, LLC at 4200 North Career Avenue, Sioux Falls, SD 57107: On or about and at times prior to June 4, 2025, the exposing employer did not ensure that each employee installing roof shingles on a new multi-family residence with the use of a pneumatic nail gun was protected from flying particles by the use of appropriate eye protection.  This condition exposed employees to struck-by hazards and eye injuries.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a)S&T Construction, LLC at 4200 North Career Avenue, Sioux Falls, SD 57107: On or about and at times prior to June 4, 2025, the exposing employer did not ensure that employees were protected from falls while conducting roofing operations on a new multi-family residential roof. This condition exposed employees to fall hazards greater than six feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)S&T Construction, LLC at 4200 North Career Avenue, Sioux Falls, SD 57107: On or about and at times prior to June 4, 2025, the exposing employer did not ensure that the ladder the employees were using was extended at least three feet above the upper landing surface of the north garage gable end of a new multi-family residential roof.  This condition exposed employees to fall hazards.
