29 CFR  1926.960(c)(2)(i): When an employee used rubber insulating gloves as insulation from energized parts, the employer did not ensure that rubber insulating sleeves were also used:  On or about June 2, 2025, at times prior thereto, the employer did not ensure employees who worked on single phase electrical distribution line used rubber insulating gloves and rubber insulating sleeves.

29 CFR  1926.960(c)(2)(ii)(A): The employer did not ensure rubber insulating gloves and sleeves were put on in a position where he or she cannot reach into the minimum approach distance, established by the employer under paragraph (c)(1) of this section:    On or about June 2, 2025, and at time prior thereto, the employer did not ensure employees put on insulated sleeves prior to entering the minimum approach distance while working on or near single phase electrical line.
