29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  (a)Lineage Logistics, LLC at 5050 West Foundation Court, Sioux Falls, SD 57107:  On or about and at times prior to June 9, 2025,  the employer did not ensure that an employee had successfully completed the training required prior to operating a powered pallet jack.  This condition exposed employees to crushing, caught-in and pinch hazards.
