29 CFR  1910.178(l)(3)(i)(A): Powered industrial truck operators did not receive training on operating instructions, warnings, and precautions for the types of truck the operator was authorized to operate, incorporated by reference in 29 CFR  1926.602(d): Powered industrial truck operator training:   (a) 215 Oberlin Road, Middletown, PA - On or about June 10, 2025, the employer did not ensure that employee(s) who changed out the LP gas tank on the Komatsu forklift (serial # 560042A) were trained to ensure proper placement of the tank to relieve strain on the gas line.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
