29 CFR 1926.451(c): Criteria for supported scaffolds. 29 CFR 1926.451(c)(2)(i): Footings shall be level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement. On or about June 24, 2025, the employer did not ensure that the pump jack scaffold was installed as required. Employees were permitted to work on a pump jack scaffold system, approximately 14.0 feet above a lower level, and the mud sills were not adequately supported on a flat, stable surface.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled. On or about June 24, 2025, the employer did not ensure that the pump jack scaffold was installed as required. Employees were permitted to work on a pump jack scaffold system, approximately 14.0 feet above a lower level, and the poles were not secured to the structure at the base.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: 29 CFR 1926.454(a)(1): The nature of any electrical hazards, fall hazards and falling object hazards in the work area; On or about June 24, 2025, The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Employees working at approximately 14.0 feet or more above a lower level were subject to fall hazards due to the setup of the pump jack scaffold. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
