29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about July 2, 2025, the employer did not ensure that a program for the control of hazardous energies for employees tasked with service and maintenance had been established. Employees within the facility are tasked with the service and maintenance of equipment in the facility including but not limited to, automatic chucker machines, automatic bar machines, drill presses, and bench grinders. Hazardous energies present on the equipment include but are not limited to mechanical, electrical, pneumatic, and hydraulic.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about July 2, 2025, the employer did not ensure that effective machine guarding was in place on the automatic bar machines located within the facility. Employees operate the Warner and Swasey 5 spindle automatic bar machine without effective guarding in place. Employees were observed operating the equipment with the doors open exposing them to the moving machine components of the equipment. b) On or about July 2, 2025, the employer did not ensure that effective machine guarding was in place on the automatic chuck machines (warner and swasey's) located within the facility. All chuck machines located in the facility lacked effective guarding such as but not limited to interlocked door mechanisms and barrier guarding. Machines 1018, 1071, 1072, 103, 1038, 1AC, and 10 AC all contained broken, bypassed, or missing interlocked barrier guarding mechanisms. Employees operate the equipment with the door open exposing them to the moving machine components of the equipment.
