29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above the lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a) On or about June 27, 2025, at the residential property located at 255 4th Ave S, Naples, FL, 34102, employees were exposed to an approximate 20-feet fall hazard while performing roofing work in a residential property without being protected from falling with a conventional fall protection system.  CFS Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1675944, citation number 2, item number 1, and was affirmed as a final order on 12/22/2023, with respect to a workplace located at 2327 Cottman Avenue, Philadelphia, PA 19149 .  CFS Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1645433, citation number 1, item number 1, and was affirmed as a final order on 05/08/2023, with respect to a workplace located at 641 Sunrise Blvd, Lehigh Acres, FL, 33971.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:   a)  On or about June 27, 2025, at the East side of the residential property located at 255 4th Ave S, Naples, FL, 34102, employees were exposed to an approximate 10-foot fall hazard while performing roofing work using a ladder that was not secured at its top to a ridgid support.  b)   On or about June 27, 2025, at the South side of the residential property located at 255 4th Ave S, Naples, FL, 34102, employees were exposed to an approximate 10-foot fall hazard while performing roofing work using a ladder that was not secured at its top to a ridgid support.  c)  On or about June 27, 2025, at the South side of the residential property located at 255 4th Ave S, Naples, FL, 34102, employees were exposed to an approximate 20 -foot fall hazard while performing roofing work using a ladder that was not properly secured at its top to a ridgid support.
