29 CFR 1926.416(a)(1):The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: (a) Jobsite: On or about June 27, 2025, employees were not protected from electrical hazards of an uninsulated single phase 23kv overhead electric line while working to install metal roofing fascia around the exterior of a commercial building. One of the employees was carrying a piece of metal roofing fascia and inadvertently contacted the 23kv overhead electric line. In doing so the employee suffered an amputation to part of his left leg and two fingers.

29 CFR 1926.416(a)(3):Before work is begun the employer shall ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer shall post and maintain proper warning signs where such a circuit exists. The employer shall advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. (a) Jobsite: On or about June 27, 2025, the employer began work without determining if any overhead electric lines presented a hazard to employees who had to install roofing metal around the perimeter of the building. On June 27, 2025, employees were installing metal roofing fascia around the perimeter of the building and came in contact with an uninsulated overhead single phase 23 KV electric line. The employer did not post any warning signs on the working level of the roof to warn employees of the overhead energized electric line.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets (a) Jobsite: On or about June 27, 2025, employees were not protected from fall hazards while installing metal roofing fascia around the perimeter of a commercial building. Employees were working on a flat roof in order to install metal roofing fascia trim while in the process without safety monitor, guardrails or any other sort of fall protection was in use by the employees.
