29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 300 Meyers Road, Liverpool, NY, accessing the roof, on or about 7/2/25:  Workers were performing roofing work on a 5:12 pitch roof without a positive means of fall protection.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   a) 300 Meyers Road, Liverpool, NY, on or about 7/2/25:  A worker fell when accessing a roof surface, 24 feet from ground, was injured and hospitalized.  The employer failed to notify OSHA about the hospitalization.
