29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Job site, 1231 Western Avenue, Pittsburgh, PA: On or about July 8, 2025 during gutter repair activities, the employer did not ensure employees used appropriate fall protection systems/devices and where exposed to fall hazards of approximately 15 feet.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards:   a) Job site, 1231 Western Avenue, Pittsburgh, PA: On or about July 8, 2025 during gutter repair activities, the employer did not ensure an employee was provided with appropriate fall protection training prior to working at heights where fall hazards of approximately 15 feet existed.
