29 CFR 1910.244(a)(2)(iii): Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack: On or about June 27, 2025, the employer failed to ensure the Larco 20-Ton Bridge Crane was cribbed, blocked or otherwise secured after crane was lifted for a wheel change by a Milwaukee M18 Force Logic 10,00PSI hydraulic lift and associated components exposing employees to a crushing hazard. The lack of cribbing or blocking of the crane, failure of the hydraulic lift and associated components, and falling of crane onto track after being lifted resulted in a partial finger amputation of an employee.

29 CFR 1910.244(a)(2)(vi)(a): Each jack used constantly or intermittently at a single locality was not thoroughly inspected at least once every six months: On or about June 27, 2025, the employer did not ensure the Milwaukee M18 Force Logic 10,000 PSI hydraulic pump and associated components used intermittently was thoroughly inspected at least once every six months. This led to a failure in the hydraulic pump and associated components during a wheel change of a Larco-20 Ton Bridge Crane resulting in an employee partial finger amputation.
