29 CFR  1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a) Located at 6547 Roberts Road, Hilliard, OH 43026: On or about July 11, 2025, the employer did not conduct frequent and regular inspections of the job site. As a result, employees were exposed to fall hazards greater than 16 feet while performing residential construction.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) 6547 Roberts Road, Hilliard, OH 43026: On or about July 11, 2025, the employer failed to ensure that each employee exposed to a fall hazard was trained on the hazards of falling and the procedures to minimize the hazards in the work area. As a result, employees were exposed to fall hazards greater than 16 feet while performing commercial construction.

The employee was exposed to a fall height of greater than 16 feet.

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a) Located at 6547 Roberts Road, Hilliard, OH 43026: On or about July 11, 2025, employees were framing a new commercial facility without the use of proper anchorage on an aerial boom lift, thereby exposing employees to fall hazards of approximately 16 feet.
