29 CFR  1910.178(m)(1):Trucks shall not be driven up to anyone standing in front of a bench or other fixed object.  (a) On or about July 21, 2025, the employer did not insure steps were taken to prevent the Toyota forklift (Model: 8FG50U, Serial: 11014) being used to roll heavy gauge steel duct work from driving up to employee(s) standing next to or working near the stationary mixer. This exposed employee(s) to a crushing hazard.

29 CFR  1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged:  (a) On or about July 21, 2025, the employer did not ensure employees operating the Toyota forklift (Model: 8FG50U, Serial: 11014) secured the heavy gauge steel duct work to the forks, in that the employees put the straps onto the forks and lifted the duct at a 45-degree angle without securing the straps. This exposes employee to a struck-by hazard.
