29 CFR1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.  Location: 136 Gracey Avenue Meriden, CT 06451  On or about July 30, 2025, employees were exposed to slip, trip and fall hazards in the liquid recycling facility (LRF) when the lower level walking/working surface was extremely slippery from a recent chemical tank overflow and an active condensate pipeline leak.

29 CFR1910.101(b):  The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6.  Location: 136 Gracey Avenue Meriden, CT 06451  On or about July 30, 2025, and times prior to, the employer had not ensured that seven (7) nitrogen gas cylinders were stored in accordance with the Compressed Gas Association Pamphlet P-1-1965, Safe Handling of Compressed Gases, section 3.3 Storing Cylinders. The cylinders were not chained/secured in place during use and/or storage.  Tradebe Environmental Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.101(b), which was contained in OSHA inspection number 1808819, citation number 1 item number 1 and was affirmed as a final order on June 30, 2025, with respect to a workplace located at 5611 West Hemlock Street Milwaukee, WI 53223.

Location: 136 Gracey Avenue Meriden, CT 06451  On or about July 30, 2025, and times prior to, the employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirement of this standard were being followed for machinery such as (but not limited to) the Tank F-4 and Tank R-4 pump motors.
