Facility, 161 Westec Drive, Mount Pleasant, PA 15666: On or about August 7, 2025, an employee was not safely positioned while troubleshooting SCM Accord-40 fx-m axis CNC router (Model No. SAW5ORBSBXSC200) Serial (00299389). The employee sustained an amputation of the right index finger from the exposed cooling fan.
