29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person:  On or about August 13, 2025, employees were exposed to struck by hazards while performing construction activities, including working next to a concrete wall with tools and material placed on the edge because the employer did not ensure a competent person conducted frequent and regular inspections of the worksite and equipment to identify the workplace hazards.

29 CFR 1926.501(c)(3): Barricade the area to which objects could fall, prohibit employees from entering the barricaded area, and keep objects that may fall far enough away from the edge of a higher level so that those objects would not go over the edge if they were accidentally displaced:  On or about August 13, 2025, employees were exposed to struck by hazards while performing construction activities, including waterproofing the underground tunnel next to a concrete wall, because tools and material were placed on the edge and the employer did not barricade the area.
