29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury associated with work within excavations:  (a)Horizontal Boring & Tunneling Company at East Arrowhead Parkway and Veterans Parkway, Sioux Falls, SD 57110:  On or about and at times prior to August 15, 2025, the employer did not ensure that an employee was adequately trained on trenching and cave-in hazards prior to working within a excavation. This condition exposed an employee to cave-in and struck-by hazards.
