At a jobsite located at 655 SE Wood Lane, Lees Summit, MO 64063, employees were exposed to eye and face injuries from flying particle hazards. Employees conducting residential roofing work were observed using pneumatic nail guns to install roofing materials without eye and face protection.

Employees were observed working at heights greater than 6 feet to the next lower level without having adequate fall protection while conducting various residential roofing work.

Employees were using a portable extension ladder where the side rails did not extend at least 3 feet past the upper landing surface to access/egress and move materials on/off the roof during residential roofing activities.

Employees were using a gasoline powered air compressor to power pneumatic nail guns used to conduct roofing work. The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work. The hazardous chemical used at the site was gasoline.
