29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 15205 Hills Rd, Kearney MO -- employees engaged in roofing duties using pneumatic nail guns, hammers and other various small hand tools were not wearing/using any eye protection. This exposed employees to getting shrapnel and/or debris in their eyes causing injury. This was documented on or about 08/19/2025.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the worksite located at 15205 Hills Rd, Kearney MO -- employees were gaining access to the roof for roofing duties via an extension ladder that did not extend 3 feet above the upper landing surface (roof). This exposed employees to fall hazards resulting in serious injury and/or death. This was documented on or about 08/19/2025.
