29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) TSK Partners, Inc. dba McInnes Rolled Rings - On or about 9/11/25 and times prior thereto, employees did not lockout prior to servicing operations on equipment including but not limited to, 1402 Preform Press. Employees are periodically required to perform tool changes on the press. Employees were trying to remove tooling at the time of an amputation injury. b.) TSK Partners, Inc. dba McInnes Rolled Rings - On or about 9/11/25, and at times prior thereto, employees are not always locking out equipment prior to conducting the servicing and maintenance activities. This equipment includes but is not limited to mechanical power presses. Employees performed service/maintenance tasks that included but were not limited to, preventative maintenance, repairs and tool changes. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
