29 CFR  1910.178(l)(2)(ii):Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) In the yard, on the site  located at 24226 Production Cir, Bonita Springs, FL : On or about 09/10/25,  the employer exposed employees to struck by hazards, in that forklift operators are not certified using the proper combination of instruction.

29 CFR  1910.178(q)(7):Industrial trucks were not examined before being placed in service.  a) In the yard, on the site  located at 24226 Production Cir, Bonita Springs, FL : On or about 09/10/25,  the employer exposed employees to caught in between hazards, in that the forklift operator did not examine the forklift emergency brake prior to being placed in service.
