29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  On or about October 8, 2025, at the jobsite located at 445 Windy Point Drive, Glendale Heights, Illinois, the employer did not develop and utilize a procedure to ensure that employees unplugged cord-and-plug machines prior to performing servicing and maintenance activities.  An employee attempted to service a storopack table without first unplugging it, exposing them to laceration and amputation injuries.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
