29 CFR 1910.132(d)(1)(i):When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard: Location: Truck unloading port, rear of facility On or about 10/1/25, the employer did not ensure that a third party driver was wearing a face shield as part of the offloading process for a liquid material that was 160 or greater degrees Fahrenheit. The employee was sprayed with the liquid material when a hose failed, and the employee experienced burn injuries.
