On or about November 19, 2025, at a workplace located at 850 Stanton Street, Ripon, Wisconsin in the production areas; the employer did not develop, document and utilize energy control procedures when employees were performing servicing and/or maintenance tasks, including but not limited to, repairs, cleaning and/or setup activities. The equipment had multiple energy sources, including but not limited to electrical, mechanical and/or pneumatic energy sources exposing employees to entanglement and/or shear hazards.
