29 CFR  1910.212(a)(3)(ii):  Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  On or about November 18, 2025, the employer did not ensure that employees operating the Trumpf V320 brake press were protected from the point of operation. During press operations, an employee engaged the foot switch to power the Trumpf press and reached beyond the unguarded point of operation during a cycle of the press with their hand.  With no point of operation guard in place, the result was a crushed hand injury and partial finger amputation of the employee.
