29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service. At the jobsite located at 13902 Norby Road, Grandview, MO 64030, employees were exposed to hazards associated with the utilization of powered industrial trucks without first checking their operational status. This was most recently documented on December 9th, 2025, when the employer allowed an employee to operate a powered industrial truck without first performing daily inspections.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: At the jobsite located at 13902 Norby Road, Grandview, MO 64030, employees were exposed to electric shock and fire hazards in that the grounding pin of various pieces of equipment were missing. The missing grounding pin was observed during the following instances: 1. The orange extension cord connected to machine 1 2. The black cord connected to machine 7 3. The hot glue gun in the shipping department.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. The strain relief issues were observed during the following instances: 1. The orange extension cord/ flexible cable connected to machine 1 - The orange flexible cable was used to connect various hand held tools. 2. The black flexible cable connected to machine 7 - The black flexible cable powered the hopper atop the machine.
