29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (a) On or about December 4, 2025, in building 1 on the Harlan Lamination "hotline", the employer did not provide employees devices or hardware to isolate/lock out all of the energy sources, when conducting maintenance/repairs on equipment. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about December 4, 2025, in building 1 on the H-2 "hotline", the employer did not ensure that employees utilized lockout/tagout procedures to control potentially hazardous energy while performing servicing and maintenance activities on machinery and equipment, including, but not limited to, the Harlan board coater. Patrick Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i):, which was contained in OSHA inspection number 1687855, citation number 1, item number 1 and was affirmed as a final order on September 25, 2023, with respect to a workplace located at 20 Eby Chiques Road, Mount Joy, PA. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
