29 CFR 1926.28(a):Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: On or about December 10th, 2025, employees engaged in electrical operations located at 100 Centennial Mall, N, Lincoln, Nebraska 68508, were exposed to electrical shock in that an employee was working on an energized 277 Volt circuit without electrically rated gloves. As a result of not utilizing proper PPE, the employee was shocked by 277 Volts.

29 CFR 1926.416(a)(1):The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: On or about December 10th, 2025, employees engaged electrical operations located at 100 Centennial Mall, N, Lincoln, Nebraska 68508, were exposed to electrical shock in that employees were working on an energized 277 volt circuit without additional safety measures in place such as guarding, LOTO, or other available methods.

29 CFR 1926.417(a):Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: On or about December 10th, 2025, employees engaged in electrical operations located at 100 Centennial Mall, N, Lincoln, Nebraska 68508, were exposed to electrical shock in that employees were working on an energized 277 Volt circuit and the equipment was not de-energized or tagged/LOTO. As a result, employee was shocked by a 277 volt energized circuit.
