29 CFR 1910.119(e)(6): The employer did not update and revalidate the requirements in paragraph (e)(4) of this section at least every five years, to assure that the process hazard analysis is consistent with the current process:  At a worksite located at or near 2429 East 535th Road, Buffalo, MO 65622:  Employees were exposed to the following hazards that included, but were not limited to fire, explosions, toxic releases, major spill, and runaway chemical reactions. The employer failed to ensure that the current process at the facility was consistent with the process hazard analysis (PHA). This was demonstrated on or about November 26, 2025, and at times prior to when the PHA was not updated and revalidated every five years.

At a worksite located at or near 2429 East 535th Road, Buffalo, MO 65622:  Employees were exposed to the following hazards that included, but were not limited to fire, explosions, toxic releases, major spill, and runaway chemical reactions. The employer failed to ensure that employees involved in the operation process were being trained at least every three years on the current process at the facility. This was demonstrated on or about November 26, 2025, and at times prior to when the employers last documented training was March 14, 2022.

The employer failed to establish and implement a written respiratory program for voluntary/required use of respirators. This was demonstrated when the employer had employees using respirators as part of the manufacturing process due to exposure to powdered based chemicals.
