29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   A) Hong Kong Foam U.S. DBA HealthCare Cargo - 45 Patrick Ave., Edison, NJ. 08837 - On December 15, 2025, an employee was exposed to caught-in and amputation hazards at the tail roller of a conveyor. The employer failed to implement adequate machine guarding, resulting in the employee's arm being pulled into the unguarded conveyor and causing an injury to the employee's right forearm.  Incident occurred on December 15, 2025
